Outreach Services

Outreach services are provided to residents who may be experiencing difficulties or barriers in accessing community services.  Issues may include accessing or maintaining housing, financial stresses, accessing food or clothing, single parenting concerns, or family transitions.  Outreach services can provide referrals, short-term support and problem sovling to address such life issues.

There are a few steps in the outreach process.  First community residents in need of support are identified.  Residents may call or visit personally, be referred by friends or family, or be recommended by other service professionals.  Needs and issues are then discussed with residents, usually through a home visit.  Together, the resident and the Community Outreach Coordinator will set goals in order to work toward what is most important.  Residents are linked to community resources as needed.  In-home support is provided when necessary.
